I had posted the first two lines earlier as well. The third line is inspired from a Nida Fazli's poem and I think it fits well to form a Triveni.

Teri yaad ganga mein baha aaya hoon
Jism se rooh ko judaa kar aya hoon

Kabhi ana tum mere qabr par fateha padhne
